Truckin’ Movers Corporation Note A Page 1 of 2 Intrastate Move Protection Although Truckin’ Movers Corporation (TMC) will make every effort to carefully work around your premises and to handle your possessions with skill and care, loss or damage can occur. We offer three levels of move protection covering your premises and loss or damage to your household goods. Option 1.“Minimal value” protection: We do not charge extra for this protection option. It pays at a maximum of $100 per occurrence of damage to premises, and a maximum of $0.60/lb per item for damage to household goods, the amount contingent upon the weight of the lost or damaged items, or repair cost on same, whichever is less. Option 2. “Depreciated value” protection: This option will pay the full cost of repairing or replacing any lost or damaged article up to an amount equal to the current normal market price for comparable new replacements, less depreciation. Option 3.”Replacement value” protection: This option will pay the full cost of repairing or replacing any lost or damaged article up to an amount equal to the current normal market price for comparable new replacements. There is a $0.00 deductible at all times for moves taking place entirely within North Carolina. Should a fire or collision occur, the free protection (option 1) would not be extensive enough to cover a total loss situation. For this reason most of our customers choose to purchase either option two or three. Please be aware you may be covered by only one of the three options. THE FOLLOWING ARE NOT COVERED BY ANY OF THE THREE OPTIONS: A. Loss or damage to documents, currency, money, jewelry, watches, art, precious stones or articles valued at more than $100/pound, unless they are written up on a special “high value” inventory by Truckin’ Movers or its duly authorized agent with a written declaration of their values before they are handled. B. Loss or damage from insects, moths, vermin, ordinary wear and tear and/or gradual deterioration, inherent vice, depreciation and/or delay, and changes in atmospheric conditions. C. Loss or damage caused by breakage of fragile articles, unless packed by Truckin’ Movers or its duly authorized agent and a charge is made and paid for such service; or unless such loss is directly caused by fire, collision, overturn, tornado, cyclone, windstorm, or flood; or unless packed in original unopened container; or unless those cartons clearly bear evidence of abusive or negligent handling and the contents of the carton have been adequately wrapped with cushioning material. D. Furs or garments trimmed with fur. E. Live plants. F. The emotional or sentimental value of any object. G. Loss or damage as set forth in the War Risk Exclusion or the Nuclear Exclusion Clause of typical insurance policies. H. TMC is liable only for item actually lost or damaged. Matched pieces, pairs or sets have no effect on the value of a settlement. I. The mechanical or electrical derangement of television sets, stereo components, computers, refrigerators, washing machines, etc., unless clearly caused by the move, with new external damage to the item evident. J. Truckin’ Movers personnel shall not drain waterbeds, remove mattresses or liners from standing waterbeds, or prepare mattresses or liners for shipment. Nor shall TMC personnel reinstall mattress or liner at delivery. If asked to do so, TMC shall not assume responsibility for any damage that could occur to a mattress or liner. K. Spoilage of contents of deep freezers, however caused. L. Loss or damage contributed to in any way by customer or by customer supplied helpers. M. When TMC provides a labor only service, loading a customer’s vehicle, damage caused during transit or handling by other than TMC personnel is not covered. When figuring the cost for the damage protection that you select, please understand that YOU MUST PROTECT YOUR ENTIRE SHIPMENT FOR AT LEAST ITS “MINIMUM TOTAL VALUE”. The minimum total value (MTV) for options 2 and 3 are as follows: Option 2 (Depreciated) - MTV = $1.25 per lb. of cargo weight (multiply 1.25 times the weight of the shipment) Option 3 (Replacement) - MTV = $4.00 per lb. of cargo weight (multiply 4.0 times the weight of the shipment) If your shipment is not weighed, TMC will determine a weight for valuation purposes by multiplying the number of cubic feet your shipment occupies by 7.0 lbs/cubic foot. Once you have estimated either the depreciated or the replacement value for your entire shipment, use the rate chart below to determine your cost for move protection. The rates are per $100 of declared value. To determine your rate, divide the total value of your shipment by $100, then multiply the result by the appropriate rate. For example, $10,000 of option 3 cargo coverage would cost $75. Option 2 Rate: .50 RATES ARE Option 3 Rate: .75 PER $100 OF VALUE Failure on the customers part to comply with the following will sacrifice customers right to collect on a claim: A. Reporting damage to concealed items: Claims on damages to concealed items (in boxes) must be reported to TMC in writing within nine months of the delivery date. If inventory forms were drawn up for the move TMC will need the inventory number along with the initials of the packer (located on the top of carton). B. Reporting lost items or damage to unconcealed items: Any claims for missing items or for damages to unconcealed items (not in boxes) must be properly noted on the inventory forms, if inventory forms were drawn up for the move, or on the billing form for shipments that do not have inventory forms, and be signed by the delivering crew leader representing TMC and by the customer. C. Responsiveness during claims process: Should you have a claim resulting from your move and TMC requests information from you and you fail to respond within sixty days to that request your claim may be considered “stale” resulting in forfeiture of your right to collect on the claim. I______________________, choose to release my entire shipment of goods as initialed below. If I leave the value space for my choice blank TMC shall set the minimum total value for my shipment as described above.
